Membership Information

The annual membership fee is $4200 per year, payable annually or quarterly. Children under 26 are included for no extra fee. The fee for adults age 27-33 is $2100 per year.

Please note: Except for the annual physical, members are charged for each office visit and for additional services. We’ll collect your insurance co-pay when you visit and bill your insurance. You may receive an invoice for any remaining balance.

The annual fee does not include care provided by other physicians, hospitalization, medications, or any other outside services. Office visits in addition to the annual physical will be billed to your insurance company.
